Please make sure to fill in all of the areas. For example, make sure to include your old email and your new one if you were changing emails. However, if you are only changing emails and not websites, then, say, you don't need to fill in the section 'New Website.' Basically, fill in all of your old info regardless, and then for the 'new' categories, only put things that have changed.
